Going Home

Your physician or hospitalist will write an order for discharge when he or she determines that you are ready to return home. You will want to make arrangements with a family member or friend for transportation. Or, you may make special arrangements with our medical social worker or discharge planner for transportation if you are going to another facility.

When you are ready for discharge, a member of your healthcare team will give you instructions for your care. This is also the time for you to ask any questions you may have about medications, activities, diet, etc.

Don't Forget Your Personal Belongings

Check your room, including the closet and drawers, to be sure you have everything that belongs to you. If you asked our Security Team to store some things for you, don't forget to call them before leaving.

We'll See You Safely On Your Way

We will escort you from your room to the hospital entrance when you are ready to leave the hospital.

A Plan For Your Recovery

A member of the hospital staff will visit you before you leave to assist you in planning for any additional follow-up care you may need. There are times when patients need rehabilitation services or extra help at home. Culpeper Regional Hospital (CRH) has licensed, certified caregivers and programs that may be helpful to you.
