Patient Role in Safe Care
You may be the most important member of the healthcare team when it comes to safety. Let us know how we're doing. Tell us what we can do better. Ask questions about anything you don't understand.
Speak Up
Be actively engaged in your healthcare!
Speak up if you have questions or concerns, and if you don't understand, ask again. It's your body and you have a right to know.
Pay attention to the care you are receiving. Make sure you're getting the right treatments and medications from the right healthcare professionals. Don't assume anything.
Educate yourself about your diagnosis, the medical tests you are undergoing, and your treatment plan.
Ask a trusted family member or friend to be your advocate.
Know what medications you take and why you take them. Medication errors are the most common healthcare errors.
Use a hospital, clinic, surgery center, or other type of healthcare organization that has undergone a rigorous on-site evaluation based on established state-of-the-art quality and safety standards, such as that provided by The Joint Commission.
Participate in all decisions about your treatment. You are the center of the healthcare team.
Medications
If you are a patient at Culpeper Regional Hospital (CRH), be sure to tell us about all of your medications. The best way to do this is to bring a list with you when you come to the hospital. Your list should include the name of your medications, the dose that you take, and how often you take it. If you don't have an accurate list, just bring all of your medicines with you. We can't always get in touch with your regular doctor or pharmacists, so we need your help to know this information.
When you leave, be sure you understand which medicines you should be taking when you go home. Ask as many questions as you need so that you understand as much as possible about the medications you are taking. If you are not able to get a new prescription filled, please be sure to let us know. Failing to take the correct medications or to take your medications correctly could result in needing to return to the hospital.
Care Journal
Take notes while you are in the hospital. It can be hard to remember the questions you want to ask and the information you are given. Write everything down so that you can remember it later. You can ask us to spell the long medical terms for you! To help you, we will offer you a care journal where you can write your questions and notes about important things your doctors and nurses tell you.
