Advance Directives

The Right To Choose

Culpeper Regional Hospital (CRH) recognizes and supports your right to decide when, how, and if care will be provided. Advance directives are the means by which you can let it be known, in advance of any care or treatment, what your wishes are.

Under Virginia law two types of advance directives are recognized:

  • A living will.
  • Appointment of someone to make healthcare decisions on your behalf in the event that you're unable to speak for yourself.

A living will serves as a testament to your instructions with regard to whether or not you forbid or permit use of life-extending procedures that would only delay the natural progression of a terminal illness.

Appointing someone to represent your wishes means designating an individual to make healthcare decisions for you in the event that your illness or injury prevents you from making these decisions for yourself.

You may also use advance directives to:

  • Set forth consent to a person on your behalf to make an anatomical gift.
  • Establish limits for your declared agent.

We encourage you to consider the use of advance directives to allow for your wishes to be known should you succumb to a condition that does not permit you to express your wishes.

The following form is presented for information purposes only. Please discuss this matter with family members prior to requesting CRH or other agency assistance in formalizing advance directive documents.
